Essential Duties and Responsibilities:

Summary:

Summary:

Summary: The GBHI Housing/Benefits Coordinator will link participants with public benefits and help them manage and retain housing. The Housing/Benefits Coordinator will work closely with local and government agencies to enroll participants in Medi-Cal, SSI/SSDI, Covered California health insurance, General Relief, and any other benefits to which the individual may be entitled. Providing advocacy to resolve enrollment needs will be ongoing responsibility, troubleshooting daily housing issues via advocacy with housing managers and systems. - Complete initial resource and services assessment for each participant
- Design a service plan for each participant

Primary Objectives:
The Care Coordinator is responsible for using a collaborative and coordinated approach to linking participants with appropriate health and social services to address their specific needs and treatment goals. - Assists the primary counselor with discharge-planning efforts, commencing at the time of admission on all assigned participants, including housing, employment, medical/psychiatric, financial, aftercare, etc.
- Provides participants with high-quality referrals and linkages for patients to necessary resources and services as identified in the Treatment Plan/Problem List, playing an active role in reducing access barriers and ensuring participants have ‘actual’ access to needed services.
- Acts as a bridge between health and human service providers to ensure that information is appropriately exchanged, and participants are successfully linked to needed resources/services.
- Coordinates care with other community agencies by assisting participants with eligibility, documentation, and applications, etc. needed to access services/resources.
- Assists participants with transitions within the SUD system of care, referring to higher or lower level of care as appropriate.
- Follows up with participants to ensure all scheduled appointments and linkages are completed.
- Collaborates and communicates fully with other team members regarding participant behavior and provides weekly updates of participant’s progress.
- Routinely identifies new and existing community resources that participants may benefit from.
- Conducts outreach to the community to develop relationships with other providers and organizations that can benefit program participants in supporting their recovery.

Summary:
The Program Coordinator supports the activities of a Program Director or Designee, which provides high-quality substance abuse recovery services for people who are struggling with addictions to alcohol and other drugs by assisting them in achieving identified goals. The Program Coordinator provides person-centered counseling and coordinated case management to adult clients toward removing barriers to substance abuse treatment and addressing housing, employment, education, financial and medical and mental health needs. TB test are to be conducted under licensed medical supervision not more than 60 calendar days prior to or 7 calendar days after employment and renewed annually from the date of the last tuberculosis test. Staff with a known record of tuberculosis or record of positive testing shall not be required to obtain a tuberculosis skin test. Unless there is documentation that the staff completed at least 6 months of preventive therapy, the staff shall be required to obtain, within 45 calendar days of employment, a chest x-ray result and a physician’s statement that the incumbent does not have communicable tuberculosis and has been under regular care and monitoring for tuberculosis. A chest x-ray within the prior 6 months is acceptable. The physician’s statement shall be renewed annually.

Summary: The Licensed/License Eligible Therapist will spend the majority of his/her time providing individual therapy to clients from residential to outpatient care.
Responsibilities and Duties:
- Individual and group therapy
- Biopsychosocial assessments, utilization reviews, discharge summaries, treatment plan reviews, and other documentation for clients
- Case management as necessary for assigned clients
- Crisis and clinical support to programs as needed
- Individual and group therapy at both Pico and Conscious Recovery locations
- Biopsychosocial assessments, utilization reviews, discharge summaries, treatment plan reviews, and other documentation for clients
- Meet with insurance clients to conduct utilization reviews and document medical necessity
- Attend treatment team meetings and have treatment team notes ready for each client
- Conduct case conferences with team members as necessary
- Meet timeframes for documents, treatment plans, etc.
- Case Management
- Crisis and Clinical support to Programs as needed
- Assist programs with clinical and mental health needs as they arise
- Conduct crisis intervention sessions and assess for client stability; suicide and/or Tarasoff assessment may be needed
- Provide support to Clinical services as needed
Knowledge, Experience & Skills:
The ideal candidate will have a strong understanding of the client care landscape. The following are strongly desired:
- Licensed or license eligible by BBS as LMFT or LCSW or Licensed by BOP as Psychologist required
- Experience with mental status exams and diagnoses
- Responsive to feedback and a team player while also being able to work independently
- Potential on-call or evening hours
- Rate based on experience
