CLARE|MATRIX is required by law to make sure that a client’s protected health information is kept private. All treatment clients will be asked to provide a signed acknowledgement of a Notice of Privacy Practices as a requirement of the Health Insurance Portability and Accountability Act (HIPAA).
All donor information, such as addresses/telephone numbers, gift amounts and beneficiaries, shall be kept strictly confidential by CLARE|MATRIX, its staff and volunteers, unless permission is obtained from donors to release such information.
